Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to oversee the timely completion and submission of MDS assessments, CCAs, and care plans in accordance with CMS RAI guidelines. You will work with an interdisciplinary team to ensure accurate documentation and regulatory compliance.

The role requires RN licensure, an associate or bachelor's degree in nursing, and at least two years of nursing experience, preferably in geriatric care.

Qualifications

• Active Registered Nurse license in good standing.
• Two years of nursing experience; geriatric experience preferred.
• CPR certification at/after hire and kept current.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS assessments, CCAs, and care plans.
• Report changes in a patient’s condition identified by the MDS assessment to the DON.
• Educate direct care staff on CMS RAI updates and facility regulations.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review Final Validation Reports and ensure CMS QIES submissions are ready for billing.
