Apple Rehab Laurel Woods in East Haven, CT seeks an RN MDS Coordinator to lead the MDS department, assist in discharge planning, and assess short-term residents for optimal outcomes.

The ideal candidate will maximize Medicare reimbursement and ensure Medicare compliance, coordinating with interdisciplinary teams to establish concise, effective care plans for each resident and to meet regulatory requirements.

Qualifications

• Must hold a current state RN license and be a nurse in good standing.
• Must meet all applicable federal and state licensure requirements.
• Attention to detail, good follow through skills and ability to prioritize multiple tasks.
• Ability to instruct others.
• Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices, procedures, laws, regulations and guidelines governing long term care.

Responsibilities

• Leads the MDS department and coordinates discharge planning for residents.
• Gathers information, assesses needs, and develops organized care plans with interdisciplinary input.
• Ensures care plans are used in daily nursing services and reviews their effectiveness.
• Plans, schedules weekly care plan meetings in accordance with OBRA and state requirements.
• Completes the MDS with accuracy and ensures CMS Mega Rule compliance; PCC experience is a plus.
