Ideal candidates hold RN licensure, with PPS/OBRA experience and prior LTC or hospital exposure. Strong computer skills and accuracy in assessments are essential for monthly close readiness.

Qualifications

• Holds current RN licensure in state of practice.
• Experience with PPS and OBRA assessments.
• Previous hospital or long-term care experience preferred.
• Knowledge of nursing, rehabilitative and restorative practices and related laws.
• Experience completing MDS and Care Plans is required.

Responsibilities

• Direct the Resident Assessment Process and assist with MDS/CAA completion.
• Develop individualized resident Care Plans and determine required outcomes.
• Perform MDS/RAI Care Plan functions and complete designated items.
• Participate in resident Care Plan Meetings.
