Qualifications

• RN licensure in the practice state and PPS/OBRA assessment experience.
• Ability to complete MDS items and Care Plans.
• Detail-oriented with timely, accurate assessments.

Responsibilities

• Direct the MDS/RAI process and assist with completing MDS and CAA.
• Develop individualized resident Care Plans and identify needs and outcomes.
• Complete nursing MDS items, CAAs and Care Plans as designated.
• Participate in resident Care Plan Meetings.
