Job summary

CareOne at Wilmington is seeking an MDS/Clinical Reimbursement Coordinator to join our long-term care team. The role requires an RN licensure with PPS/OBRA experience and the ability to manage MDS and Care Plans efficiently.

Qualifications

• RN licensure in MA with PPS/OBRA experience.
• Experience in hospital, long-term facility, or related healthcare.
• Knowledge of long-term care regulations and MDS/Care Plans.
• Proficient with Windows; AccuMed experience preferred.

Responsibilities

• Direct the Resident Assessment Process and assist with completing MDS/CAA.
• Develop individualized resident Care Plans and identify needs and outcomes.
• MDS/RAI Care Plan Functions.
• Complete MDS items, CAAs and Care Plans as designated by the facility.
• Participate in resident Care Plan Meetings.
