Responsibilities include data entry, CMS submissions, and communication with residents and families. The role requires strong assessment, documentation, and interdisciplinary teamwork skills within a long-term care setting.

Qualifications

• Graduate of an accredited school of nursing with an active RN license.
• 2–3 years of clinical nursing experience; 1 year in long-term care preferred.
• Experience with MDS coordination and regulatory compliance is preferred.
• Strong organizational, communication, and documentation skills.

Responsibilities

• Coordinate the completion and submission of all MDS assessments per regulations.
• Oversee data entry of MDS into the EHR and verify accuracy before CMS submission.
• Collaborate with care teams to develop and update individualized care plans.
• Participate in care plan meetings and ensure changes reflect resident needs.
• Ensure regulatory compliance and support reimbursement efforts.
