Arlington Pointe Care Center in Middletown, OH seeks an experienced MDS Coordinator RN to manage the MDS assessment process in a skilled nursing facility, ensuring timely, accurate assessments and regulatory compliance.

You will collaborate with nursing, therapy, dietary and social services to build individualized care plans, oversee MDS data entry in the EHR, and support reimbursement through precise CMS submissions.

Qualifications

• Graduate of a nursing program with RN licensure and active state license.
• Minimum 2–3 years of clinical nursing experience, with long-term care preferred.
• Experience coordinating MDS assessments and familiarity with MDS 3.0 and RAI.

Responsibilities

• MDS assessment coordination and submission per regulations.
• Data entry and QA of MDS within the facility's EHR system.
• Collaborate with interdisciplinary team to create care plans.
• Update care plans and ensure implementation.
• Maintain regulatory compliance and CMS submissions.
• Communicate with residents and families about MDS processes.
• Participate in team meetings to review progress and adjust plans.
• Support quality improvement initiatives and audits related to MDS.
