LionStone Care is seeking an MDS Coordinator - RN to manage MDS assessments and care planning for residents at Arlington Pointe Care Center in Middletown, OH. The role ensures regulatory compliance and optimal reimbursement through accurate data submissions.

Ideal candidates hold an RN license with an Associates Degree in Nursing and have 2–3 years of clinical nursing experience, including long-term care. Strong collaboration with interdisciplinary teams is essential.

Formación

• Graduate of an accredited school of nursing with an active RN license.
• RN licensure active and unrestricted.
• 2–3 years of clinical nursing experience, with at least 1 year in long-term care or skilled nursing. MDS coordination preferred.

Responsabilidades

• MDS Assessment Coordination: coordinate completion and submission of all MDS assessments per regulations and timelines.
• Care Planning: collaborate with interdisciplinary team to create individualized care plans.
• Regulatory Compliance: maintain knowledge of MDS, RAI, and Medicare/Medicaid reimbursement; ensure CMS submissions.
