The role emphasizes accurate clinical documentation, regulatory compliance, and strong communication. You will ensure timely MDS completion and collaborate on person-centered plans that reflect each resident’s needs and goals.

Qualifications

• Active RN license in the United States.
• RAC-CT certification required.
• Long-term care experience required.
• Current CPR certification.
• Strong organizational, communication, and clinical documentation skills.

Responsibilities

• Coordinate the completion of accurate and timely MDS resident assessments.
• Develop and update individualized care plans in collaboration with the interdisciplinary team.
• Review clinical documentation to support accurate assessments and care planning.
• Help ensure assessment and care-planning processes meet applicable regulatory requirements.
• Communicate resident needs and changes to support coordinated, quality care.
