Job summary

The Orchards is currently seeking an MDS Coordinator to join our team in Harper Woods, Michigan. You will be responsible for completing accurate assessments, MDS and care plans, and initiating and supporting care plan activities for residents.

You will also create monthly care plan calendars, update all plans as required, and monitor clinical records for accuracy and timeliness while collaborating with the DON and participating in relevant committees.

Qualifications

• Valid RN or LPN License.
• Knowledge of skilled nursing facilities survey process and regulations would be helpful.
• Must have prior MDS Experience.
• Long Term Care Experience Required.
• Must be able to work 40 hours a week.
• Must be highly organized, professional & motivated.
• Should have solid computer skills.
• Excellent communication skills.
• Should be friendly and a team worker.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining & updating all care plans and assessments as required.
• Monitoring & auditing clinical records, ensuring accuracy & timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident & Facility information at all times.
• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.
