Job summary

Spring Grove Rehabilitation and Healthcare Center is seeking an MDS Coordinator to lead accurate MDS assessments, ensuring regulatory compliance and timely submissions for resident care planning and reimbursement.

The role requires an active RN license, at least 3 years in long-term care, and prior MDS/RAI experience, with strong regulatory knowledge and collaboration with care teams. In-person, full-time position with competitive benefits.

Qualifications

• RN license active and in good standing
• Minimum of three years of clinical experience in a long-term care setting
• Prior MDS/RAI experience required
• Strong understanding of Medicare/Medicaid regulations and reimbursement

Responsibilities

• Ensure timely and accurate completion of all MDS assessments in accordance with regulatory requirements and deadlines
• Verify compliance with federal, state, and facility regulations related to MDS documentation and submission
• Supervise MDS data entry, validation, and transmission processes
• Identify, investigate, and resolve data discrepancies and validation issues
• Prepare and present MDS-related reports to the Director of Nursing and leadership
• Collaborate with interdisciplinary teams to ensure accurate resident care representation
• Support facility surveys, audits, and regulatory reviews with required documentation
