Job summary

Phoenix Rehabilitation and Nursing Center is seeking a skilled MDS Assessor (RN) to join our team. The role focuses on completing the Resident Assessment Instrument and coordinating services with physicians, families, and payers in a timely manner.

Responsibilities include overseeing MDS/CAA/care plans, identifying regulatory risk, communicating with the IDT, and ensuring timely submission of MDS data. This on-site position offers a competitive salary and equal opportunity employment.

Qualifications

• Registered Nurse with current, active license in state of practice.
• Minimum of one (1) year of experience in a long term care setting.
• Training program available for RN candidates with demonstrated assessment skills.

Responsibilities

• Oversees accurate and thorough completion of the MDS, CAAs and Care Plans in accordance with regulations.
• Acts as in-house Case Manager coordinating with physicians, families, and third party payers.
• Communicates regulatory risk and resource capture to Administrator and DON.
• Utilizes ARD and ensures correct RUG score and payer mix.
• Ensures timely electronic submission of all MDS to the state database.
• Supports IDT in discharge planning and staff education on the RAI process.
