Job summary

National Health Care Associates is seeking an MDS Coordinator / Nurse Assessment Coordinator to ensure accurate MDS assessments for residents and contribute to personalized care plans. The role supports accurate reimbursement and encourages high standards of care within a Great Place to Work Certified environment.

You will collaborate with interdisciplinary teams, monitor CMI, and ensure regulatory compliance while guiding facility staff through MDS processes and education.

Qualifications

• RN nursing license valid in the state.
• Advanced degree or certification preferred.
• Experience in long-term care and MDS coordination preferred.
• Knowledge of MDS, PDPM, EMR, Medicaid and Medicare requirements helpful.

Responsibilities

• Determine PDPM and associated admission expenses.
• Assess nursing needs and determine reimbursement levels for admissions.
• Complete and assure accuracy of the MDS process for all residents.
• Monitor Case Mix Index (CMI) scores for potential risks to reimbursement.
• Ensure revenue integrity and compliance with regulations for MDS coding.
• Collaborate with interdisciplinary teams to ensure accurate data collection.
