Job summary

Stonebridge at Montgomery is seeking an experienced MDS Coordinator to join our team and coordinate the RAI process, ensuring accuracy, timeliness, and completeness of MDS, CAAs, and care plans. A $5,000 sign-on bonus is available for this full-time on-site role in Skillman, NJ.

The MDS Coordinator will oversee MDS assessments, ensure federal and state regulatory compliance, and support the interdisciplinary team in delivering high quality resident care.

Qualifications

• Must possess a current Nursing license in the state in which the MDS Coordinator is working.
• RAC Certification must be obtained within one year of hire.
• ICD-10 Coding Certification (AANAC) required within one year of hire.
• 3 years supervisory experience required.
• Experience in LTC preferred.

Responsibilities

• Determine eligibility of Medicare residents and communicate to nursing personnel the required documentation.
• Coordinate the RAI process with the interdisciplinary team.
• Oversee and facilitate completion of MDS resident assessments and ensure regulatory compliance.
• Submit MDS to CMS and review validation reports for corrections.
• Participate in Resident Care Plan development and evaluation.
• Assign and order ICD-10 codes according to resident condition.
