Job summary

Lake Wales Wellness & Rehabilitation Center is seeking a dedicated MDS Coordinator to join our care team in Lake Wales, Florida. The role requires RN licensure in Florida, strong MDS/RAI knowledge, and supervisory nursing experience.

You will attend weekly education meetings, coordinate the RAI process, and complete MDS assessments while collaborating with the interdisciplinary team to evaluate and update care plans. This is a full-time position with competitive benefits.

Qualifications

• Active RN license in Florida.
• Minimum of 3 years nursing experience including supervisory responsibilities.
• MDS training must be completed within 6 months of hire.
• RAC certification preferred.

Responsibilities

• Attend weekly education meetings to stay updated on MDS changes.
• Coordinate the facility’s Resident Assessment Instrument (RAI) process per guidelines.
• Accurately complete MDS assessments and supporting documentation.
• Evaluate residents' care plans and audit medical records with the interdisciplinary team.
• Perform additional assignments from Regional MDS Consultants, Administrator, and Director of Nursing.
• Maintain confidentiality of protected health information in all forms.

MDS Coordinator Position Summary

As a MDS Coordinator (LPN or RN) at Lake Wales Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Attend weekly educations meetings to stay updated on MDS changes.
• Coordinate the facility’s Resident Assessment Instrument (RAI) process in accordance with state and federal guidelines.
• Accurately complete all MDS assessments and any supporting assessments or clinical documentation.
• Evaluation of resident’s comprehensive plan of care, auditing medical records for supporting documentation, collaborating with the interdisciplinary team.
• Perform any other additional tasks as assigned by the Regional MDS Consultants, Administrator, and Director of Nursing.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

MDS Coordinator Requirements

• Active RN license for the state of Florida
• 3 years nursing experience including supervisory experience
• MDS training must be completed within 6 months of hire
• RAC Certification preferred
