Job summary

Cordova Wellness & Rehabilitation Center is seeking a dedicated MDS Coordinator (LPN or RN) to ensure accurate resident assessments and compliant documentation. You will manage the RAI process, review care plans, and collaborate with the interdisciplinary team to optimize resident outcomes.

This full‑time role offers a supportive work environment, paid time off, health coverage, and a $5,000 sign‑on bonus. An active Tennessee LPN or RN license is required; PDPM/CMI experience is preferred.

Qualifications

• Active Tennessee LPN or RN license required.
• 2–5 years experience with PDPM, CMI, and Quality Measures.
• RAC Certification preferred.

Responsibilities

• Attend weekly education meetings to stay updated on MDS changes.
• Coordinate the facility’s Resident Assessment Instrument (RAI) process per state and federal guidelines.
• Accurately complete all MDS assessments and supporting documentation.
• Evaluate resident’s care plans and audit medical records with the interdisciplinary team.
• Perform tasks assigned by Regional MDS Consultants, Administrator, and Director of Nursing.
• Maintain confidentiality of protected health information in all formats.

MDS Coordinator Position Summary

As a MDS Coordinator (LPN or RN) at Cordova Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Attend weekly educations meetings to stay updated on MDS changes.
• Coordinate the facility’s Resident Assessment Instrument (RAI) process in accordance with state and federal guidelines.
• Accurately complete all MDS assessments and any supporting assessments or clinical documentation.
• Evaluation of resident’s comprehensive plan of care, auditing medical records for supporting documentation, collaborating with the interdisciplinary team.
• Perform any other additional tasks as assigned by the Regional MDS Consultants, Administrator, and Director of Nursing.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

Requirements

• Active LPN or RN license for the state of Tennessee
• 2-5 Years Experience with PDPM, CMI, and Quality Measures
• RAC Certification preferred
