Job summary

Carecore-Healt is seeking an experienced MDS Coordinator to join a dedicated long-term care team committed to resident care excellence. This onsite role focuses on accurate assessments, regulatory compliance, and quality outcomes within an interdisciplinary setting.

You will collaborate with nursing, therapy, dietary, social services, and other team members, coordinate MDS assessments, and support documentation, education, and survey readiness to drive care quality and reimbursement integrity.

Qualifications

  • Active LPN or RN license required. RN preferred.
  • Previous MDS Coordinator experience in a long-term care or skilled nursing setting required.
  • RAC-CT certification preferred.
  • Working knowledge of Medicare, Medicaid, the RAI process, and PDPM.
  • Strong assessment, organizational, and critical thinking skills.
  • Experience with PCC preferred.
  • Excellent communication and teamwork abilities.

Responsibilities

  • Complete and coordinate MDS assessments in accordance with state and federal regulations.
  • Ensure accurate and timely completion of care plans and required documentation.
  • Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.
  • Monitor clinical documentation to support appropriate reimbursement and regulatory compliance.
  • Participate in quality improvement initiatives and survey preparedness.
  • Educate and support staff regarding MDS processes and documentation requirements.
