Job summary

Aviata Health Group is seeking a qualified MDS Coordinator (LPN) or (RN) to join our Lake Mary, Florida location. You will oversee the coordination and completion of the Resident Assessment Instrument (MDS, CAA’s, and Care Plan) in accordance with Federal and State Regulations.

We are a family/team oriented employer dedicated to compassionate care. Ideal candidates have an active LPN/RN license and at least 6 months of MDS Coord experience, with RAC-CT or RNAC preferred.

Qualifications

• Must possess a current, active state license as an LPN or RN.
• Six (6) months of experience as a MDS Coordinator.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• RAC-CT or RNAC certification preferred.

Responsibilities

• Oversee the coordination and completion of the MDS, CAA’s, and Care Plan per Federal and State Regulations.
• Monitor and document Medicare and Managed Care resident management in collaboration with facility team members.
• Contribute to improvements in systems and processes to achieve better outcomes.

Job Summary:

Aviata Health Group is seeking a qualified MDS Coordinator (LPN) or (RN) to join our team at our Lake Mary, Florida location. As a key member of our team, you will be responsible for overseeing the coordination and completion of the Resident Assessment Instrument (MDS, CAA’s, and Care Plan) in accordance with current Federal and State Regulations. Job Responsibilities:

• Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA’s, and Care Plan) in accordance with current Federal and State Regulations.


• Monitor and document the management of the Medicare and Managed Care residents in collaboration with facility team members.


• Provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results.

Qualifications:

• Must possess a current, unencumbered, active state license to practice as an LPN or RN.


• Experience in Skilled Nursing/Rehabilitation facilities preferred.


• Six (6) months of experience as a MDS Coordinator.


• RAC-CT or RNAC preferred.


• You must be qualified, compassionate, and dedicated to a job well done.

Compensation:

• LPN: $37 - $40 hour


• RN: $89,440 - $95,680
