Qualifications & Experience

• Graduate of an accredited nursing school; RN license required
• Valid RN license in the state of employment
• Preferred: Three years of experience in a long-term care setting
• Experience with MDS/RAI process and case management is a plus

Job Responsibilities

The MDS Nurse RAC (Resident Assessment Coordinator) reports to the Executive Director and is responsible for the accurate and timely completion of MDS assessments and coordination of the RAI process. This role oversees Medicare, Medicaid (case mix), and managed care to ensure appropriate clinical services and reimbursement. The position ensures compliance with federal and state regulations, as well as company policies. The MDS Nurse is a vital part of the facility’s management team, helping to achieve and maintain quality outcomes.
