RN Mobile MDS Coordinator, Registered Nurse

Position Overview

CommuniCare Health Services is seeking a Mobile MDS Coordinator, RN for its facilities in the Eastern Panhandle region of West Virginia. The ideal candidate must hold a valid RN license in West Virginia and possess experience in Long Term Care and MDS. Multi-facility or Senior MDS experience is preferred. The role requires attention to detail, teamwork, effective communication with residents, families, and staff at all levels, and travel across the covered regions.

Position Requirements

1. Current RN license in West Virginia
2. Prior Long Term Care experience
3. At least 2 years of experience managing Federal Medicare/Medicaid and Managed Care reimbursement systems, including MDS and RAI processes
4. Senior MDS and/or multi-facility MDS experience preferred
5. AANAC certification highly preferred
6. Excellent written and verbal communication skills
7. Ability to multi-task and oversee multiple facilities daily
8. Current CPR certification

Responsibilities

1. Evaluate facility compliance with federal regulations and utilization targets
2. Conduct weekly visits to facilities for intervention, audits, and training
3. Review resident clinical records for coverage, utilization, errors, and assessments
4. Provide consultation on federal and state reimbursement systems
