Registered Nurse (RN), Mobile MDS Coordinator

CommuniCare Health Services is seeking a dedicated Registered Nurse to serve as a Mobile MDS Coordinator across our skilled nursing facilities in the Midwest Division. The ideal candidate must hold a valid RN license in Ohio and possess prior experience with Long Term Care and MDS processes. Experience with multi-facility MDS or senior MDS roles is preferred. The Mobile MDS Coordinator should be detail-oriented, a strong team player, and capable of effective communication with residents, families, and staff at all levels. This role involves travel throughout Ohio and occasionally to Indiana, with facilities located in Cincinnati, Dayton, Columbus, Akron, Youngstown, Cleveland, and Toledo areas.

POSITION REQUIREMENTS

• Current Ohio RN license
• Minimum 2 years' experience with Federal Medicare/Medicaid and Managed Care reimbursement systems, including MDS and RAI processes
• Preferred: Senior MDS experience and/or multi-facility MDS experience
• Highly preferred: AANAC certification
• Excellent written and verbal communication skills
• Ability to multi-task and oversee multiple facilities daily
• Current CPR certification

Responsibilities

• Evaluate facility compliance with federal regulations and utilization targets
• Conduct weekly visits for intervention, audits, and training
• Review resident clinical records for coverage, utilization, errors, assessments, and transmission accuracy
• Provide consultation on federal and state reimbursement systems
