About the Opportunity:
As an MDS Coordinator, you will play a crucial role in ensuring accurate and timely completion of resident assessments and care planning. Your expertise will directly impact the quality of care we provide and ensure compliance with regulatory requirements.
Key Responsibilities:
Complete and coordinate the Minimum Data Set (MDS) assessments and care plans in accordance with state and federal regulations.
Collaborate closely with the interdisciplinary team to develop individualized care plans that meet the needs of each resident.
Ensure timely submissions and maintain documentation to support billing accuracy and regulatory compliance.
Serve as a resource and support for staff regarding care plans, assessments, and resident documentation.
Participate in Quality Assurance and Improvement (QAPI) activities to continually enhance resident care outcomes.
Qualifications:
Active RN License in the State of Ohio – Required.
Prior experience with MDS 3.0 and care planning processes preferred.
Strong knowledge of federal and state regulations related to long-term care and MDS reporting.
Excellent organizational, communication, and clinical assessment skills.
