Job summary

Ascension Living in Signal Mountain, TN is seeking an RN to lead MDS development and care planning in the Skilled Care department at Alexian Village of Tennessee.

You will ensure MDS accuracy, uphold regulatory standards, and coordinate CMS survey readiness in collaboration with interdisciplinary teams to optimize resident care.

Qualifications

• RN license required prior to hire date or transfer date.
• Diploma from an accredited school/college of nursing required or professional licensure at time of hire.
• One year of RN-MDS experience preferred.

Responsibilities

• MDS & Care Plan Accuracy: ensure precision of charts, data, and assessments while training team members to develop comprehensive care plans.
• Regulatory & Program Compliance: stay current on Medicare/Medicaid/ licensure/ accreditation requirements for long-term care.
• Survey Preparedness & Teamwork: maintain survey readiness and collaborate across disciplines for all CMS survey phases.
