Qualifikationen

• Must hold Tennessee nursing license (RN or LPN).
• Strong reading, writing, grammar, mathematics, interpersonal and communication skills.
• Ability to manage multiple priorities and time effectively.
• Proficient computer skills including Word, Excel and MDS software.

Aufgaben

• Coordinate with DON/ADON to minimize use of resident restraints.
• Assess resident care needs and develop care plans and MDS.
• Track falls and report QA findings.
• Coordinate with Social Services and Nursing to complete PAE/PASRR.
• Collaborate with Admissions to create financial reports for new admissions into Skilled rehab.
• Monitor resident medications and reduce psychotropic meds.
• Coordinate with MCOs for payment approvals and appeals.
• Input admission and discharge data into MDS.
• Facilitate patient care plan meetings and notify families.
• Ensure accuracy of input data for electronic submission.
• Maintain privacy and dignity of residents and comply with HIPAA.
