Uplands Village in Pleasant Hill, TN is seeking a registered nurse or licensed practical nurse to join our long-term care team. The position requires nursing licensure in Tennessee, strong reading, writing, and math skills, and the ability to manage multiple priorities in a fast-paced environment.

You will work with the Clinical Compliance Director, coordinate with the DON/ADON, monitor restraints, MDS data input, patient care planning, and QA reporting.

Qualifications

• Graduate of an approved school of nursing with Tennessee RN or LPN license.
• Post-offer, pre-employment physical completed.
• Ability to manage multiple priorities.

Responsabilités

• Coordinate with DON/ADON to monitor restraints use.
• Determine nursing care needs to meet resident care plan and MDS.
• Track falls and report QA findings.
• Input admission and discharge data into MDS.
• Facilitate patient care plan meetings and notify families.
• Ensure input data accuracy for electronic submissions.
• Maintain resident privacy and HIPAA compliance.
