Job summary

Morristown Post Acute Rehabilitation & Nursing Center is seeking an RN MDS Coordinator to guide the MDS process and care planning for residents. You will collaborate with an interdisciplinary team to ensure comprehensive, individualized care that aligns with each resident’s goals and regulatory requirements.

Qualifications

• Coordinate MDS process including assessments and care plans.
• Communicate with healthcare professionals to ensure resident goals.
• Maintain compliance with federal/state regulations and facility policies.

Responsibilities

• Coordinate the MDS process, ensuring timely and accurate completion of MDS assessments and care plans
• Collaborate with the healthcare team to develop and implement individualized care plans, incorporating resident goals and preferences
• Ensure compliance with federal and state regulations, as well as facility policies and procedures
• Monitor and report changes in resident condition, and facilitate communication among healthcare providers
• Participate in quality improvement initiatives and contribute to the development of process improvements
• Demonstrate a strong commitment to resident-centered care, promoting a positive and supportive environment
