Job summary

Apple Rehab Rocky Hill is seeking a part-time MDS Care Plan Coordinator (RN) to lead the MDS department, coordinate discharge planning and assess short-term residents. The role emphasizes maximizing reimbursement while ensuring Medicare compliance within a collaborative care team.

Responsibilities include organizing interdisciplinary assessments, writing care plans, and conducting weekly meetings to align on goals. Strong attention to detail and regulatory knowledge are essential.

Qualifications

• Must hold a current state RN license and be a nurse in good standing.
• Must meet all applicable federal and state licensure requirements.
• Attention to detail, good follow through skills and ability to prioritize multiple tasks.
• Ability to instruct others.
• Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices, procedures, laws, regulations and guidelines governing long term care.

Responsibilities

• Lead the MDS department and coordinate discharge planning.
• Assess short-term residents and maximize reimbursement while ensuring Medicare compliance.
• Coordinate interdisciplinary team assessment and develop written care plans.
• Schedule and conduct weekly care plan meetings per OBRA/state rules.
• Ensure nursing staff follow care plans and monitor resident progress.
