Apple Rehab Rocky Hill is seeking a dedicated MDS Care Plan Coordinator (RN) to lead the MDS department and coordinate discharge planning for short-term residents. You will gather data, assess needs, establish goals, and integrate actions into organized care plans with input from an interdisciplinary team.

Qualifications

• Must hold a current state RN license and be a nurse in good standing.
• Knowledge of OBRA and Medicare regulations.
• Experience coordinating MDS, discharge planning, and care plans.

Responsibilities

• Leads the MDS department and coordinates short-term resident care planning.
• Gathers information, assesses needs, sets goals, and writes concise care plans.
• Schedules weekly care plan meetings and ensures plans are followed by nursing staff.
