Job summary

CommuniCare Health Services seeks an RN MDS Nurse to join our team. The MDS Nurse coordinates MDS assessments, oversees the RAI process, and ensures appropriate clinical services and reimbursement for residents.

The role reports to the Executive Director and supports quality outcomes across the facility. Graduate of an accredited school of nursing with an active RN license in the state of employment is required; three years of long-term care experience and MDS/RAI familiarity are preferred.

Qualifications

• Graduate of an accredited school of nursing.
• RN license valid in the state employed.
• Three years of experience in a long term care environment preferred.
• Experience with the MDS/RAI process and/or case management preferred.

Responsibilities

• Complete accurate and timely MDS assessments and coordinate the RAI process.
• Provide Medicare, Medicaid (case mix), and managed care oversight to ensure appropriate clinical services and reimbursement.
• Ensure compliance with federal and state regulations and CommuniCare guidelines and policies.
