The ideal candidate is an MN-licensed RN with strong assessment skills, attention to detail, and the ability to collaborate with the admissions team to ensure accurate documentation

Qualifications

• RN licensed in Minnesota with current practice credentials.
• Thorough understanding of MDS assessment process and Medicare/Medicaid regulations.
• Excellent clinical assessment and interpretation of resident needs.
• Strong communication and interpersonal skills with residents, families, and team.

Responsibilities

• Conduct thorough MDS assessments to evaluate functional abilities and care needs.
• Document resident information in the MDS system and identify care concerns.
• Analyze MDS data to develop individualized care plans with goals and interventions.
• Monitor progress and update MDS assessments as conditions change.
• Review applications and medical records to determine suitability for admission.
• Perform initial assessments with residents and families to discuss needs.
• Coordinate tours and answer questions with the admissions team.
• Handle admission paperwork, medical records transfer, and room assignment.
