Job summary

Signature HealthCARE’s Monroe County Rehab & Wellness Center seeks an experienced MDS Coordinator (RN) to lead resident assessments and coordinate the MDS process. You will collect information, complete sections, and ensure compliance with federal billing and care plans.

Ideal candidate has RN license, at least 3 years of clinical healthcare experience, and a minimum of 1 year MDS experience, with ability to educate staff on RAI processes and collaborate with the interdisciplinary team.

Qualifications

• RN license with current state credentials.
• Minimum three years of clinical experience in healthcare; long-term care preferred.
• Minimum one year of MDS experience and knowledge of RAI processes.

Responsibilities

• Collect information to complete the MDS using the medical record, bedside assessment, and staff, resident and/or family interviews.
• Participate in PPS meetings, Medicare meetings, and month-end meetings to ensure federal billing requirements are met.
• Provide ongoing education of the RAI process to the interdisciplinary team and caregivers.
