Job summary

What We’re Looking For…

We are seeking an experienced RN MDS Coordinator to join our team! The RN MDS Coordinator will lead the MDS process for our skilled nursing facility ensuring accurate and timely MDS assessments, Care Plans, ICD-10 coding, GG documentation, and Medicare PDPM reimbursement in accordance with state and federal regulations. This position coordinates care plan conferences, collaborates with nursing, therapy, billing, residents, and families, and oversees required documentation and Physician Certifications. The RN MDS Coordinator will also monitor Quality Indicators, participate in QAPI initiatives, maintain current knowledge of regulatory requirements, and provide ongoing education and guidance to staff.

Candidates must hold a current RN license in Florida, be a graduate of an accredited nursing program, and have long-term care experience, including at least two years of MDS/Care Plan experience. Strong knowledge of MDS, PDPM, geriatric nursing, and regulatory requirements, along with excellent computer, communication, organizational, and problem-solving skills, is required.
