Job summary

HC&N Healthcare Solutions is seeking a skilled MDS Coordinator (RN) to join our team at a skilled nursing facility in Florida. You will oversee MDS, CAA, and Care Plans in line with federal and state guidelines.

You will work with the resident, IDT, and family to identify barriers to discharge, analyze monthly QI/QM trends with the DON, and ensure timely Utilization Review. Prior MDS coordination experience is preferred.

Qualifications

• Registered Nurse with active Florida license.
• 2+ years of clinical experience in a health care setting.
• 1+ year of experience in a long-term care setting.
• Prior MDS coordinator experience preferred.

Responsibilities

• Oversees accurate and thorough completion of the MDS, CAA's and Care Plans per regulations.
• Understands MDS requirements for Medicare, Managed Care and Medicaid.
• Facilitates the Care Management Process with resident, IDT and family for timely discharge.
• Analyzes QI/QM data with the Director of Nursing to identify trends monthly.
• Responsible for timely, accurate Utilization Review and Triple Check.
• Serves on and attends facility committees as required.
