Job summary

Delmar Center for Rehabilitation and Nursing is seeking an experienced MDS Coordinator in New York to coordinate MDS assessments, CAAs, and care plans within CMS guidelines. You will collaborate with an interdisciplinary team to ensure accurate documentation and compliant submissions.

The ideal candidate holds an active RN license, 1–2 years of MDS experience in long-term care, and demonstrates strong assessment and organizational skills to support superior resident outcomes and regulatory

Qualifications

• Active RN license in good standing.
• Minimum 1–2 years MDS experience in skilled nursing.
• Knowledge of CMS guidelines and HIPAA compliance.
• Excellent assessment, organizational, and communication skills.

Responsibilities

• Coordinate and complete MDS assessments per CMS guidelines and facility policies.
• Ensure timely completion of MDS, CAAs, and care plans.
• Collaborate with interdisciplinary team to develop and update resident care plans.
• Monitor clinical documentation to support MDS coding accuracy and reimbursement.
• Review resident records and conduct assessments to determine care needs.
• Track assessment schedules and maintain deadlines for submissions.
• Assist with Medicare/Medicaid and insurance reviews as needed.
• Educate nursing staff regarding documentation requirements and regulatory compliance.
• Monitor quality measures and identify opportunities for clinical improvement.
• Assist with survey preparedness and regulatory audits.
• Collaborate with facility leadership to ensure optimal reimbursement and resident outcomes.
• Maintain confidentiality of resident information per HIPAA regulations.

MDS Coordinator Major Tasks and Responsibilities:

• Track assessment schedules and maintain compliance with required submission deadlines.
• Participate in Medicare, Medicaid, and insurance reviews as needed.

MDS Coordinator Required Education / Experience:

• Current Registered Nurse (RN) license in good standing.
• Minimum of 1-2 years of MDS experience in a skilled nursing or long-term care setting.
• Strong knowledge of MDS 3.0, RAI process, and Medicare/Medicaid regulations.
• Excellent assessment, organizational, and communication skills.
• Ability to work independently and manage multiple deadlines.
