Elevance Health in California is seeking an LTSS Service Coordinator-RN Clinician to manage member care plans across physical health, behavioral health, social services and long-term supports. The role includes in-person assessments and collaboration with a multidisciplinary team to optimize outcomes.

The position requires an RN with at least 3 years of relevant experience, a valid license, and the ability to travel to worksites.

Qualifications

• RN with minimum 3 years of experience in coordinating care for individuals with chronic illnesses or disabilities.
• Current, unrestricted RN license in applicable state(s) required.
• State law or contract may require state-specific certification.

Responsabilités

• Supervise and direct non-RN clinicians participating in the member's case.
• Develop, monitor, evaluate, and revise the member's care plan to optimize health across the care continuum.
• Perform face-to-face clinical assessments for identification, evaluation, and management of member needs.
• Interface with Medical Directors, Physician Advisors and Inter-Disciplinary Teams on care management plans.
