Job summary

Molina Healthcare is seeking a Nurse Care Manager to provide LTSS-focused care coordination and member support across multiple counties in Ohio. The role collaborates with a multidisciplinary team to ensure member progress and cost-effective, quality care.

Responsibilities include conducting assessments, managing waiver processes, developing care plans, and monitoring outcomes. Travel up to 40% is required in line with state contracts.

Qualifications

• At least 2 years in health care, with at least 1 year in care management or LTSS.
• Active RN license in state of practice.
• Valid driver's license and reliable transportation for travel.
• Experience with disabilities, chronic conditions or SUDs preferred.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits when required.
• Manage waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and the team.
• Monitor and document care plan effectiveness and progress toward goals.
• Coordinate services across behavioral health and LTSS for continuity of care.
• Identify barriers to care and provide coordination support for psycho/social, financial, and medical needs.
