Careforth, Inc. seeks an RN Care Manager or LSW Care Manager to onboard new families and provide ongoing caregiver support. You will assess consumer and caregiver needs, determine eligibility, and guide activation of services with a person-centered plan of care.

Regular home visits and coaching are expected. The role requires mentoring caregivers on best practices, collaborating with the clinical team, and ensuring regulatory compliance.

Qualificações

• Active RN license and/or LSW/LISW in Ohio.
• Minimum two years of clinical experience with elders or adults with disabilities.
• Ability to travel within the assigned geographic area; valid driver’s license and car insurance.
• Experience in case management, care planning, and assessment in health care.
• Ability to work in a fast-paced environment and meet KPIs.
• Tech-savvy; proficient with common software and web-based applications.

Responsabilidades

• Manage enrollment of prospective families and educate on qualifications; assess eligibility for services.
• Guide applications for Medicaid, waiver programs, and state benefits.
• Conduct initial in-person clinical assessments and coordinate service activation.
• Onboard caregivers to Careforth’s HIPAA-secure platform and ensure proper use of electronic daily communications.
• Develop and review each member’s plan of care; provide ongoing coaching and education to caregivers.
• Monitor health status and ensure needs and program requirements are delivered; coordinate with clinical teams.
