Job summary

Careforth, Inc. in the United States is seeking an Assessment RN Care Manager to guide families through the assessment process, activate services quickly, and prepare for ongoing support.

This role requires an active RN license, at least 2 years in direct elder or disability care, and ability to travel within the assigned territory. You will document care plans, ensure regulatory compliance, and support families with Careforth’s platform.

Qualifications

• Active RN license in the state of employment.
• Minimum 2 years of direct elder/adult care experience.
• Willingness and ability to travel within territory; valid driver’s license.
• Experience in case management, care planning, and assessment.
• Tech-savvy; proficient with common software and case management systems.
• Ability to work in a fast-paced environment with KPIs.

Responsibilities

• Manage initial contact with prospective families and assess service eligibility.
• Conduct initial clinical assessments onsite and guide activation.
• Orient caregivers to the Careforth app and HIPAA-secure platform.
• Document notes and help develop a care plan for start of service.
• Ensure audit readiness and regulatory compliance.
• Perform other related tasks as needed.
