Job summary

Careforth, Inc. is seeking an RN Care Manager to support caregivers and their loved ones with personalized care plans and home visits. This role emphasizes coaching via our app, resource referrals, and clinician guidance to reduce caregiver burnout. No night shifts or weekend requirements.

Qualifications

• Must hold an active RN license for the state of employment.
• Minimum two years of direct care experience with elders or disabled adults.
• Ability to travel within the assigned geographic area; valid driver's license required.
• Experience in case management, care planning and needs assessment.
• Proficiency with MS Office and case management systems.
• Additional state-specific qualifications may be required.

Responsibilities

• Conduct home visits to assess safety and health status, and coach on medications.
• Provide ongoing coaching via app, including resource referrals and best practices.
• Develop trusting caregiver relationships to reduce burnout.
• Document records for accuracy and timeliness per regulations.
• Handle other projects and duties as assigned.
• Perform other duties as assigned.
