Job summary

Careforth, Inc. is seeking an RN Care Manager or LSW/LISW Care Manager in Ohio to onboard families, assess eligibility, and guide activation of services. You will conduct home visits, develop person-centered care plans, and coach caregivers to support daily health needs.

The role requires clinical experience with elders or disabled adults, travel within the state, and strong collaboration with the clinical team while ensuring HIPAA-compliant communication and documentation.

Qualifications

• Active RN license in Ohio or LSW/LISW in Ohio is required.
• Minimum two years of direct elder/adult care clinical experience.
• Ability to travel within the assigned geographic area, with valid driver’s license.

Responsibilities

• Onboard new families onto service and guide activation of services.
• Education and coaching for caregivers on best practices.
• Conduct initial in-person clinical assessments and coordinate activations.
• Develop and review each member’s plan of care with the clinical team.
• Complete documentation for on-site visits and encounters.
• Monitor health status and ensure regulatory compliance.
• Discharge planning and transition support from the program.
