Careforth, Inc. in the United States is seeking an RN Care Manager to provide personalized care and support to caregivers and their loved ones. No night shifts or weekends; assess needs, create care plans, conduct home visits, and ensure exceptional consumer care.

Competenze

• Active RN license in the state of employment.
• Minimum two years of recent direct care experience with elders or adults with disabilities.
• Ability to travel within the assigned geographic area; valid driver’s license and auto insurance required.
• Experience in case management, care planning and assessment for complex medical needs in a health care setting.
• Proficiency in common software and web-based applications (e.g. MS Office, case management systems).

Mansioni

• Conduct home visits in the assigned territory to assess home safety and health status, and provide clinical coaching.
• Provide ongoing coaching and support to caregivers via the app, including resource referrals and best-practice reinforcement.
• Develop trusting relationships with caregivers to influence change and reduce caregiver burnout.
• Document and review records for accuracy and timeliness, ensuring regulatory and accreditation compliance.
• Perform other duties as assigned.
