Job summary

Qualifications

• Active RN license in the state of employment.
• Minimum two years' experience in direct elder or disability care.
• Ability to travel within assigned territory; valid driver’s license and car insurance.
• Experience in case management, care planning and assessment.
• Proficiency with MS Office and case management software.
• Additional state-specific qualifications may be required.

Responsibilities

• Conduct home visits to assess safety and health status.
• Provide ongoing coaching to caregivers via app and refer resources.
• Develop trusting relationships to reduce caregiver burnout.
• Document records accurately and ensure regulatory compliance.
• Handle other projects and duties as assigned.
• Perform other duties as assigned.
