Trinity Center is a 120 bed skilled nursing facility within Luther Park Community in Des Moines, IA. As a Charge Nurse, you will plan, coordinate, and provide nursing care to residents while supervising staff and assigning nursing duties as appropriate.

Qualifications

• Graduate of an accredited school of nursing.
• Current nursing license in the state of Iowa.
• CPR certification preferred or must obtain within 30 days of hire.
• One year of experience in a long term care or memory care setting preferred.

Responsibilities

• Inform the Director of Nursing (DON) and Assistant DON (ADON) of resident status and pertinent matters.
• Admit and orient new residents with family involvement.
• Review physician orders and pre-admission information; ensure accuracy.
• Supervise CNAs and CMAs; assign nursing duties.
• Maintain documentation for narcotics, charts, and weekly skin assessments.
• Monitor weights and vital signs and inform physicians of changes.
• Complete incident/investigation reports and notify relevant parties.
• Discharge residents and coordinate transfers to hospital when needed.

Full-time | 2:00 PM – 10:30 PM | Every Other Weekend | Starting at $39/Hour + Shift Differential

Luther Park Community is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Charge Nurse at Trinity Center, our skilled nursing and memory care community in Des Moines, IA.

Job Summary

Trinity Center is a 120 bed skilled nursing facility within Luther Park Community, including 60 beds dedicated to memory care.

As a Charge Nurse, you will plan, coordinate, and provide nursing care to residents while supervising staff and assigning nursing duties as appropriate. You will help ensure compliance with established policies and procedures while performing professional nursing duties within the scope of your license.

Key Responsibilities

• Keep Director of Nursing (DON) and Assistant Director of Nursing (ADON) informed of resident status and other pertinent matters
• Admit and orient new resident and involving family members with orientation
• Check physician orders, transfer forms and pre-admission information
• Review resident’s medical and social history
• Complete admission and documentation forms
• Supervise CNAs and CMAs
• Ensure residents’ supplies and medications are ordered and equipment is working properly
• Maintain proper documentation of facility records: narcotic records, medical chart forms, weekly skin assessment
• Maintain daily, weekly or monthly weight and vital signs and informs physician, DON, ADON and On Call Manager of any changes
• Complete incident reports, investigation reports and notify physician, family members, DON/ADON and On Call Manager
• Discharge residents to community or other facilities, transfers residents to hospital, completes required forms and documentation, ensures resident is escorted to transportation and contact family prior to any transfers to hospital

Graduate of an accredited school of nursing

Current nursing license in good standing in the state of Iowa
