Luther Park Community in Des Moines, IA is seeking a compassionate Licensed Practical Nurse (LPN) to join TrinityCenter as a Charge Nurse. This full-time role at TrinityCenter involves planning, coordinating, and providing nursing care while supervising staff and ensuring policy compliance.

Position details include 2:00 PM–10:30 PM shifts, every other weekend, with starting pay at $32/hour plus shift differential. Luther Park offers competitive benefits, PTO, and continued education support.

Qualifications

• Graduate of an accredited school of nursing.
• Current Iowa nursing license in good standing.
• CPR certification preferred or must obtain within 30 days of hire.
• One year of experience in a long-term care or memory care setting preferred.
• Excellent written and oral communication skills.
• Must relate positively to residents and families and work cooperatively with staff.

Responsibilities

• Keep DON and ADON informed of resident status and pertinent matters.
• Admit and orient new residents with family involvement.
• Check physician orders, transfer forms and pre-admission information.
• Review residents' medical and social history.
• Complete admission and documentation forms.
• Supervise CNAs and CMAs.
• Ensure supplies and medications are ordered and equipment works.
• Maintain facility records including narcotics and medical charts.
• Document weight and vital signs and inform physicians of changes.
• Complete incident/investigation reports and notify stakeholders.
• Discharge residents and coordinate transfers to community or hospital.

Job Summary

Trinity Center is a 120 bed skilled nursing facility withinLuther Park Community, including 60 beds dedicated to memory care.

As a Charge Nurse, you will plan, coordinate, and providenursing care to residents while supervising staff and assigning nursing dutiesas appropriate. You will help ensure compliance with established policies andprocedures while performing professional nursing duties within the scope ofyour license.

Key Responsibilities

• Keep Director of Nursing (DON) andAssistant Director of Nursing (ADON) informed of resident status and otherpertinent matters

• Admit and orient new resident andinvolving family members with orientation

• Check physician orders, transferforms and pre-admission information

• Review resident's medical andsocial history

• Complete admission anddocumentation forms

• Supervise CNAs and CMAs

• Ensure residents' supplies andmedications are ordered and equipment is working properly

• Maintain proper documentation offacility records: narcotic records, medical chart forms, weekly skin assessment

• Maintain daily, weekly or monthlyweight and vital signs and informs physician, DON, ADON and On Call Manager ofany changes

• Complete incident reports,investigation reports and notify physician, family members, DON/ADON and OnCall Manager

• Discharge residents to communityor other facilities, transfers residents to hospital, completes required formsand documentation, ensures resident is escorted to transportation and contactfamily prior to any transfers to hospital

Minimum Job Requirements

• Graduate of an accredited schoolof nursing

• Current nursing license in goodstanding in the state of Iowa
