Job summary

Luther Park Community is seeking a caring and dependable Certified Nursing Assistant (CNA) to join our team full-time at Trinity Center, our skilled nursing and memory care community in Des Moines, IA. As a CNA, you will provide personalized care and assist residents under the direction of the nursing team, helping with activities of daily living and promoting comfort and independence.

Shifts run 6:00 AM–2:30 PM and 2:00 PM–10:30 PM, with every-other-weekend work and a shift differential for the

Qualifications

• High school diploma or equivalent.
• Successful completion of the 75-hour CNA course.
• Certified Nursing Assistant certificate required.
• Valid and current on the Iowa Nurse Aide Registry.
• Ability to read, write, and comprehend English.

Responsibilities

• Provide individualized care according to each resident’s care plan and daily needs.
• Assist residents with activities of daily living including bathing, grooming, dressing, oral hygiene, toileting, and incontinence care.
• Assist residents with meals, hydration, and recording food and fluid intake.
• Safely assist residents with transfers, mobility, ambulation, and repositioning using equipment and techniques.
• Observe and promptly report changes to the nurse or supervisor.
• Maintain resident dignity, privacy, rights, and independence through respectful, person-centered care.
• Promote a safe, clean environment by cleaning equipment, changing bed linens, and maintaining resident areas.
• Follow care plans and policies to ensure quality care and resident safety.
• Collaborate with the nursing team to support residents’ physical, social, and emotional well-being.

Job Summary

Trinity Center is a 120 bed skilled nursing facility withinLuther Park Community, including 60 beds dedicated to memory care.

As a CNA, you will provide personalized care and assistanceto residents under the direction of the nursing team. You will help residentswith daily activities, promote their comfort and independence, and supporttheir physical, emotional, and social well-being.

We are currently hiring full-time CNAs for the followingshifts: 6:00 AM–2:30 PM and 2:00 PM–10:30 PM. Both shifts require working everyother weekend. The 2:00 PM–10:30 PM shift offers an additional shiftdifferential.

Key Responsibilities

• Provide individualized care according to each resident’scare plan, preferences, and daily needs
• Assist residents with activities of daily living, includingbathing, grooming, dressing, oral hygiene, toileting, and incontinence care
• Assist residents with meals, feeding as needed, hydration,and recording food and fluid intake
• Safely assist residents with transfers, mobility,ambulation, and repositioning using appropriate equipment and techniques
• Observe and promptly report changes in residents’conditions, behaviors, appetite, or other concerns to the nurse or appropriatesupervisor
• Maintain resident dignity, privacy, rights, and independencewhile providing respectful, person-centered care
• Promote a safe, clean, and comfortable environment bycleaning equipment, changing bed linens, and maintaining resident areas
• Follow individual care plans, facility policies, andestablished procedures to ensure quality care and resident safety
• Work collaboratively with the nursing team to supportresidents’ physical, social, and emotional well-being

Minimum Job Requirements

• High school diploma or equivalent
