Qualifications

• Graduate of accredited school of nursing.
• RN licensure in state of practice required.
• BLS certification required.
• Long-term care experience preferred.

Responsibilities

• Assess residents on admission and ongoing basis; develop individualized plans of care.
• Administer medications and treatments per physician orders and facility policy.
• Supervise, delegate to and provide clinical direction for LPNs and CNAs.
• Evaluate resident response and revise the care plan as needed.
• Maintain accurate clinical documentation and support survey readiness.
• Coordinate care with physicians, therapy, families and interdisciplinary team.
