Job summary

Vanderbilt Dialysis Clinic is seeking an entry‑level Home Therapies RN CAP to coordinate and deliver clinically competent care for patients undergoing home dialysis. You will participate in education, planning, and evaluation with the End Stage Kidney Disease care team in a clinic setting.

Responsibilities include training patients and Home Partners, coordinating care with physicians, and ensuring accurate documentation and adherence to CAP requirements.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Provides oversight of LPNs/LVNs, RNs, and aides as Team Leader.
• Trains and mentors staff and new hires as assigned.
• Assesses and documents patient needs; coordinates home dialysis education.
• Directs safe and effective delivery of dialysis therapy to assigned patients.
• Performs data collection and documentation of patient care activities.
• Participates in on-call rotation and travel to training sites or patient homes.
