Job summary

Vanderbilt Health Nursing is seeking a seasoned RN to support care coordination across episodes of patient care in our Nashville region. The role emphasizes reducing readmissions, coordinating transitions, and engaging patients and families in self-care planning.

As part of VHVI Nashville Clinical Staff - Team 4, you will collaborate with physicians, pharmacists, and multidisciplinary teams to ensure timely, patient-centered care across the continuum.

Qualifications

• Active RN license required.
• Bachelor's degree in Nursing preferred.
• Minimum 5 years of relevant nursing experience.

Responsibilities

• Coordinate the evaluation process for the defined patient population with the care team.
• Serve as liaison between referring offices and Vanderbilt providers.
• Collaborate with clinical teams to plan care from admission through the defined care episode.
• Support discharge planning and transition to post-acute care.
• Develop patient education materials and teach-back follow-up with patients and families.
