Job summary

Vanderbilt University Medical Center in Nashville seeks a skilled nurse coordinator to manage scheduling, admission, transfer, and consults for patients requiring varying levels of care. You will coordinate referrals from referring hospitals, clinics, urgent care, and physician offices to ensure timely and appropriate transport and bed assignment.

Responsibilities include assessing patient acuity, using clinical judgment to optimize outcomes, educating nurses and physicians on admission/transfer

Qualifications

• RN licensure to practice in the applicable state
• 5 years of relevant work experience
• Strong understanding of patient flow and hospital admission/transfer processes
• Experience educating nurses, physicians and staff on level of care and admission/transfer issues

Responsibilities

• Systematic processing of patient acuity to facilitate patient flow
• Uses critical thinking to optimize outcomes based on clinical information
• Screens admissions for appropriateness of level of care and bed assignments
• Educates staff on level of care and admission/transfer issues
• Proficient in EHR data entry, telephone and office systems while protecting PHI
• Adheres to departmental procedures and policies for referral resolution
• This role includes other duties as assigned
