Qualifications

• Must hold an active RN license in good standing.
• CCM/PCM experience preferred but not required.
• Solid clinical knowledge of chronic conditions and patient education.

Responsibilities

• Conduct onboarding calls with patients and families.
• Build individualized care plans covering symptoms and risk factors.
• Lead monthly check-ins to assess changes and coordinate resources.
• Manage care logistics: referrals, refills, and reminders.
• Source regional resources (nutrition, exercise, social support) for patients.
• Collaborate with the tech team to shape AI-enabled care delivery.
